I had melasma all over my face and like others tried all the creams out there. Then I started having IPL treatments. Had approximately 8-10 on a low setting and had excellent results.
However, one needs to stay out of the sun and wear sunscreen. I use Vanicream broad spectrum SPF 50+. It is greasy, however, after 30 minutes absorbs in to your skin. I also use moisturizer with sunscreen 30 (Philosophy Miracle cream) in addition to. I was tired of driving an hour away for the IPL 2-3 times a year maintenace and went to someone closer who did not use the machine on a low setting and my melasma came back. I went to someone else who understood what happened and told me that the settings on the machine were probably too high; she uses a 2 or a 14-15 setting on the machine. She also uses a numbing cream and cold gel before "zapping" me. It is tolerable. It takes a longer time to get rid of the melasma, however, it is safe and doesn't make it worse. I have the IPL every 4 weeks and a microdermabrasion also every 4 weeks, spacing it 2 weeks after the IPL. Once the spots have lightened significantly or are eliminated, then I go for IPL's 2-3 times a year for maintenance.
The treatments are expensive, however, ask for a reduced price since you are having many.
I pay $650 for a series of 3; it takes 2-3 series to eliminate the splotches; the microderms are $100.00 each time. Expensive, however, in the end will cost less than the creams and other items I purchased which never worked. I always wear sunscreen and a wide brimmed hat.
This year I purchased a titanium umbrella from Coolibar.com and use it at soccer games.
